X1052 crashed/bricked after connecting SFP module

Hi I have for last 2 weeks used X1052 without any problem. I have setup 2 SFP+ ports (I use them in SFP mode) as LACP to connect to ZyXEL GS1920-24 on other end. Until yesterday I use only one port with ZyXEL SFP-SX-D module without any problem (other than that I had on hand two Ubiquiti UF-MM-1G modules which X1052 didn't recognize at all - but they are working no problem on other switches).

After about 2 weeks I was confident with stability enough to put second SFP module into LACP group. At first everything worked as expected but after 4-6 minutes switch suddenly rebooted and froze on bootup (at stage where all connected ports light up and are supposed to go dark again before switch starts accepting traffic again). So I tried to unplug everything and reboot switch both with reset button on the back side and by unplugging power for 10-20 second. Every time switch froze during bootup.

Because it is production switch I didn't have time to make any more tests. I reset it to factory defaults and reconfigure it, this time with both SFP plugged in from the start, everything seem to be working again, until I looked onto "Diagnostic ->Optical Transceiver Diagnostics" page shortly after (few seconds) switch rebooted and bricked itself again and I had to reset it to factory default and from then it's running with only one SFP connected without problem.

I didn't have time to gather any more info or try to look at console, but I'm planning to try again this or next weekend, has anyone any suggestions what could be causing it or how can I best obtain all logs necessary to help solve this problem if it occurs again?

PS: Switch was upgraded to last boot/firmware: 1.0.0.25/v3.0.0.94 and after factory reset it was 1.0.0.25/3.0.0.91 so I will make upgrade again before next testing.
