6224 image issues do I need to rebuild the unit???

Hi all,

 Thanks for taking the time to look at this. I have a 6224 which was on an old version of firmware think it was 1.0.0.27 and I managed to get it to version 2.0.0.12......when I look in the graphic interface the switch has no image 2..... however it says image 2 is version 2.0.0.12 untill I select next image after reset to image 2 then it shows 14.148.70.40 ..... I also cant seem to get the latest version 3x.x.x onto the switch as it fails..... as you can see I have an error down the bottom of the following.... Do I need to completely rebuild this? Is the flash corrupted? Boot sector?  If I do were are the instructions to rebuild it? Thanks for any help...
